A conversation that ends in patients being confident &comfortable in their responsibilities and receiving matching financial assistance grants to lighten the burden.

How We Work

At the core of The Community Care Foundation’s Financial Assistance Grant Program is education, counseling, and grants that match the patients’ financial commitment towards their healthcare costs, up to 50% of the patients’ out-of-pocket costs.

As part of Community Care Foundation’s mission and vision, financial assistance applicants must meet the requirements for Community Care Foundation’s grant approval, which may include:

  1. Completion of Community Care Foundation’s educational and healthcare financial literacy curriculum.
  2. Completion of healthcare financial responsibility counseling.
  3. Applicants must also apply for charity care or financial assistance through their provider.
  4. Minimal payment towards patient financial responsibility, which may include payment equal to approved financial assistance grant.
  5. Personal health information consent authorization. 
  6. Acceptance of Community Care Foundation terms and conditions.
 

The Grant process is simple:

Qualified applicants get free access to a certified payment specialist to assist in navigating their financial responsibility for their health care obligations  

  • Submit Grant application
  • Meet grant program requirements
  • Community Care Foundation's will work with the patients’ provider to provide financial responsibility estimate and agreed payment terms
  • Patient payment and CCF matching grant is processed with the healthcare provider.
